Important Update: Provisional Report of Death Form

The Kentucky Coroners Association would like to provide an important update regarding the Provisional Report of Death form.

KCA President Jeff Ivers recently spoke with Jeff Sparks of the Kentucky Office of Vital Statistics regarding the transition to the updated form.

According to Mr. Sparks, coroners may continue using the current Provisional Report of Death form through December 31, 2026.

Beginning January 1, 2027, coroners must begin using the new VS-34 Provisional Report of Death form.

The updated VS-34 form is available through the Kentucky Coroners Association Form Center:

KCA encourages all coroners and deputy coroners to download the updated form and prepare for the transition before the end of the year.
